What are dental restorations, and why are they needed?
Fixed dental restorations are custom-made by a dental lab to cover or replace teeth. They come in three main forms:
- Crown (cap): covers the whole tooth to protect it and restore its shape.
- Bridge: replaces one or more missing teeth by anchoring to the neighbouring teeth.
- Veneer: a thin shell bonded to the front of a tooth to improve its shape and colour.
They're usually recommended for large fractures, extensive decay a filling can't handle, teeth weakened after root canal treatment, replacing missing teeth, or improving the look of a smile.
Types of fixed restorations
1. Zirconia crowns
Zirconia is a very strong, metal-free ceramic and one of the most widely used options today. Its strength makes it ideal for back teeth and bridges, and it's highly biocompatible with no grey line at the gumline. Modern multi-layered zirconia is far more translucent than older generations and is often used for front teeth too.
- Pros: very strong, fracture-resistant, metal-free, colour-stable.
- Good to know: slightly less translucent than E-max, so E-max may be preferred for the most demanding front-tooth aesthetics.
2. E-max crowns
E-max (lithium disilicate) glass-ceramic is prized for translucency very close to natural enamel, making it excellent for front teeth, single crowns and veneers.
- Pros: outstanding aesthetics, natural look, often needs less tooth reduction.
- Good to know: strong, but less so than zirconia โ not usually chosen for long bridges or heavy grinders.
3. Porcelain-fused-to-metal (PFM)
The traditional option: a metal core covered with porcelain. Still used for its strength and lower cost, but a grey line may appear at the gumline over time, and it looks less natural than zirconia or E-max.
4. Veneers
Thin shells on the front of the teeth to change colour, shape or small gaps โ the basis of a "Hollywood smile". Porcelain/E-max veneers are lab-made, stain-resistant and long-lasting; composite veneers are built directly in one visit at lower cost but stain more easily and need periodic polishing.
5. Restorations on implants
When a tooth is missing, a crown (usually zirconia) can sit on a dental implant instead of preparing neighbouring teeth for a bridge. Learn more about dental implants.
Quick comparison
| Type | Aesthetics | Strength | Best for |
|---|---|---|---|
| Zirconia | Very good | Very high | Back teeth, bridges, implant crowns |
| E-max | Excellent | Good | Front teeth, veneers, single crowns |
| PFM | Moderate | High | When budget is the priority |
| Porcelain/E-max veneers | Excellent | Good once bonded | Cosmetic front-tooth changes |
| Composite veneers | Good | Moderate | Minor changes at lower cost |
E-max vs. veneers โ what's the difference?
One of the most common questions โ and they aren't directly comparable: a veneer is a type of restoration (a shell on the front of the tooth), while E-max is a material. Veneers can be made from E-max ("E-max veneers"), and full crowns can be made from E-max too.
Zirconia vs. porcelain crowns
- Appearance: zirconia has no metal, so no grey line at the gum.
- Strength: zirconia is more resistant to chipping and fracture.
- Gum health: zirconia is highly biocompatible.
- Cost: porcelain-fused-to-metal is usually cheaper.
How to choose
- Tooth position: front teeth need top aesthetics; molars need strength.
- Your bite: grinders need stronger materials and possibly a night guard.
- Remaining tooth structure: weak or root-treated teeth may need a full crown rather than a veneer.
- Gum health: inflamed gums must be treated first for a beautiful, stable result.
- Your goals and budget: discussed openly before starting.
Frequently asked questions
Does getting a crown hurt?
Tooth preparation is done under local anaesthesia, so you shouldn't feel pain. Mild, temporary sensitivity afterwards is normal.
How long do zirconia and E-max crowns last?
With good care and regular check-ups, ceramic restorations can last many years, depending on the material, tooth position, bite and daily care.
Can crowns or veneers be whitened?
No โ ceramic colour is fixed. If you want to whiten your natural teeth, do it before choosing the shade of your restorations.
Does a tooth need a root canal before a crown?
Not always โ only if the nerve is inflamed or damaged.
